Live Webinar Date & Time: Wednesday, January 30, 2019 at 1:00pm EST

Description:

The evolution of our current healthcare system has been guided by a collection of reactive strategies to complex problems of access to care, quality and safety, and misaligned financial incentives. Population Health (Management) focuses on the broader the spectrum of stakeholders, with an emphasis on specific patient needs and challenges.. This perspective affords decision makers the opportunity to design and deliver care based on accurate predictions that emphasize consumer engagement and increased value. The migration from fee-for-service to value-based reimbursement will result in improved outcomes for patients, payers, and increase provider effectiveness and satisfaction.

Learning Objectives:

  • Learners will be able to trace the history of the development of our profligate and dangerous system of care delivery.
  • Learners will be able to define population health and give specific examples of successful population health management as a tool to deliver high value care.
  • Learners will be able to make accurate predictions about the future of care delivery with an emphasis on consumer engagement to also achieve high value.
